Brain Calcifications in Adult-Onset Genetic Leukoencephalopathies / Calcificaciones cerebrales en Leucoencefalopatias de Adultos

Leukoencephalopathies are disorders that selectively involve the white matter of the brain. Many leukoencephalopathies are associated with demyelination, which is the loss or damage of previously formed myelin. Leukodystrophy refers to inherited disorders with progressive destruction or loss of previously acquired myelin. In adults, serial MRI scans may help distinguish progressive demyelination from dysmyelination, hypomyelination, and other forms of stable leukoencephalopathy. The pattern of white matter involvement may be characteristic and help refine the diagnostic process. The presence of calcification or cystic changes may also be characteristic of particular inherited leukoencephalopathies.

Despite recent progress in molecular screening and diagnostic imaging, adult-onset genetic leukoencephalopathies are still a common cause of misdiagnosis with potentially severe consequences. Missing a genetic cause may lead to a false diagnosis of an acquired disorder with putative prescription of inaccurate treatments (eg, corticosteroids, immunosuppression) and to the absence of any offer of genetic counseling, therefore leading the patient and his or her family to not receiving the best possible chances for diagnosis and treatment .

In February 2018, our group will hold the first symposium on diagnosis and counseling in the diagnosis of leukodystrophy in adult presentation.

Las leucoencefalopatías son trastornos que afectan selectivamente a la sustancia blanca del cerebro. Muchas leucoencefalopatías están asociadas con la desmielinización, que es la pérdida o daño de la mielina previamente formada. La leucodistrofia se refiere a trastornos hereditarios con destrucción progresiva o pérdida de mielina previamente adquirida. En adultos, las exploraciones seriadas de MRI pueden ayudar a distinguir la desmielinización progresiva de la dismeinación, la hipomielinización y otras formas de leucoencefalopatía estable. El patrón de implicación de la sustancia blanca puede ser característico y ayudar a orientar el proceso de diagnóstico. La presencia de calcificaciones o lesiones quisticas  también pueden ser característicos de las leucoencefalopatías hereditarias.
A pesar de los recientes avances en la detección molecular y de imágenes diagnósticas, la leucoencefalopatía genética de inicio en los adultos sigue siendo una causa común de diagnósticos erróneos con consecuencias potencialmente graves. La ausencia de una causa genética puede conducir a un error diagnóstico  de un trastorno adquirido con presunta prescripción de tratamientos inexactos (por ejemplo, corticosteroides, inmunosupresión) y la ausencia de cualquier oferta de asesoramiento genético, lo que lleva al paciente ya la familia a no recibir mejores posibilidades de diagnóstico y el tratamiento.
